Abstract

Heart failure is a complex clinical syndrome with a high prevenance that reached 1% to 3% of the general adult population. Heart failure is a progressive disease leads to frequent hospitalization and high mortality. Its diagnosis and management are challenging. Unplanned rehospitalization due to heart failure is still high.  Prevention and controlling risk factors can reduce heart failure incidence. Patient education included enhancing self-care behavior showed positive results on improving symptoms and reducing heart failure hospitalization. Primary care physicians could help in these preventive strategies and be a part of multi-disciplinary team caring patients with heart failure and consequently improving health outcomes. This review aimed to clarify the important points about chronic heart failure.
